This website provides an educational Mental Health Guide in plain language. The guide is general educational reading about mental health, wellbeing and care options. • The questionnaire is designed solely to support access to general educational information. It does not assess eligibility for any specific treatment or clinical programme, cannot diagnose a condition, and cannot replace professional screening.
• You agree to provide truthful, accurate information when completing the questionnaire.

4. Receiving the guide

• The Mental Health Guide is provided free of charge. No payment, deposit or booking is required.
• To receive the guide, you complete the questionnaire and provide an email address. The guide is sent to that address.
• We may decline to send the guide if the information provided is incomplete or appears untruthful.

5. Educational use & your responsibility

• The guide and this website are educational. They do not provide a diagnosis, do not constitute medical advice, and do not recommend any treatment.
• Any decisions about your health or treatment belong with a qualified healthcare professional. Never stop, reduce, or change prescribed medication on the basis of anything on this website.
